Long Term Condition Reviews

A long term condition review is a type of Health Check for people with a Long Term Condition for example diabetes, asthma & stroke

If you have one of the following conditions then you will be invited for an annual review or more frequently if necessary.  The review will likely happen to coinside with the month of your birth so if you are born in August, we will invite you to book in July to be seen in August.

  • Asthma
  • Atrial Fibrilation
  • High blood pressure
  • COPD
  • Cancer
  • Cholesterol
  • Chronic Kidney disease
  • Dementia
  • Depression
  • Diabetes
  • Heart disease
  • Learning disabilities
  • Severe mental health problems
  • Non Diabetic Hyperglycaemia
  • Stroke

We will contact you shortly before your review is due.  Your review may be in two parts, the first part to check your height, weight, blood pressure and to perform any relevant blood tests.  The second part will be a review with a clinician.  Not everyone needs both parts.  We will let you know what to book for when we contact you.

The appointments are usually 20-30 minutes long, however if you have more than one problem it may be longer.

Dr Hockley offers a three year contraceptive implant insertion and removal service.  Please feel free to book an appointment to discuss this.

Baby and Childhood Immunisations

Starting from 8 weeks old, your child needs to have different vaccines to protect them as they grow up.

The surgery will contact you when your child is eligible for their first set of vaccinations.  This appointment is usually booked for after your post-natal appointment with the GP.

Once you have this appointment you will be advised by the nurse when the next set of vaccinations are due and when to book these.

Childhood Immunisation Information

Cervical Screening (previously known as smear tests)

We recommend that all women have a cervical smear following national guidelines.  All women aged 25-64 are invited for a cervical screening every 5 years to check the health of their cervix.

Invites are sent through the NHS Cervical Screening Management Service.  The service has recently changed how they invite patients.  Patients are invited through the NHS app or SMS.  If the message is not read or where an SMS cannot be delivered (including where people don’t have a recorded mobile phone), a letter will be sent to the patient as a failsafe. All invites will still direct people to book in the usual way.

NHS Health checks

The NHS Health Check is a health check-up for adults in England aged 40 to 74. It’s designed to spot early signs of stroke, kidney disease, heart disease, type 2 diabetes or dementia. As we get older, we have a higher risk of developing one of these conditions. An NHS Health Check helps find ways to lower this risk.

Our nurses run clinics for men and women of any age to help and encourage you to stay healthy. Services include dietary advice, cholesterol measurement, smoking cessation and vaccination updates.

Checks for women also include cervical smears and breast examinations.

 

Minor Surgery procedures

Some of our clinicians are trained to perform minor surgical procedures.  These are available by special arrangements.  Patients will need to be assessed before this can be booked.  Please call us to discuss.
