Our Pharmacists and Pharmacy Technicians can assist with all aspects of medication management, as well as hypertension reviews, blood pressure monitoring, and menopause support.

Social Prescribers

Our Social Prescribers support patients with a wide range of non-medical issues that may impact their health and wellbeing, including housing concerns, financial difficulties, and relationship problems. They help connect patients with local groups, services, and community activities to improve wellbeing and encourage greater involvement within the community. This support can be particularly beneficial for people living with long-term conditions, mental health challenges, loneliness, or other social difficulties.

Care Coordinators

Our Care Coordinators are divided into two dedicated teams to support different patient groups. One team focuses on supporting housebound patients, ensuring they receive the clinical care and chronic disease monitoring they require. The second team supports patients diagnosed with cancer, providing advice on available services and offering valued support throughout their treatment and recovery journey.

First Contact MSK Physiotherapist

In partnership with Solent NHS Trust, we offer a First Contact Musculoskeletal (MSK) Physiotherapy service at the surgery several times each week. Patients can receive assessment and support for a wide range of musculoskeletal conditions.

Health & Wellbeing Coach

Our Health & Wellbeing Coach works closely alongside the Social Prescribers and Mental Health Facilitator to support patients looking to improve their lifestyle. This may include guidance with weight management, increasing physical activity, reducing alcohol intake, and stopping smoking.
